Every Webelos and leader younger than 40 going to camp must, upon arrival, present a completed Class 2 Medical Form. Leaders 40 and older must have a Class 3 Medical Form. The forms must be signed by medical personnel licensed to practice medicine. Licensed medical personnel are limited to a doctor of medicine, a doctor of osteopathy, a physicians assistant, or a nurse practitioner. See the links to the proper forms below.
Each Webelos or leader is given a recheck upon entering camp, but full examinations are not available. A Webelos may use the same physical form for three years from the date signed by licensed medical personnel. All medical personnel signatures on all medical forms must be dated no more than three years prior to the last day of camp. However, a new Health History (beige-colored form) signed by the parents must be current within one year.
Leaders younger than 40 years are not required to have a yearly medical exam. Like the Webelos, their physicals are acceptable for three years from date of signature by attaching the Health History (beige-colored form). Leaders 40 and older are required to have a yearly exam by licensed medical personnel.
Adults who will be spending fewer than 72 hours in camp are classified as visiting leaders and are not required to have the physician's statement. The adult must file with the camp office a current Health History (beige-colored form) . Note: Visiting leaders may not swim unless they have a physical signed by licensed medical personnel.
Unit leaders should collect and review all medical forms prior to camp. If the unit leader has any questions concerning a Scout's medical form, the parents of the Scout should be contacted prior to camp.
