Every Webelos Scout and leader spending time at Webelos Mini-camp will need to provide the NEW Annual Health & Medical Form with PARTS A & C completed and signed by the parents or guardian.  You bring the form with you to camp.  Most Webelos leaders will collect them ahead of time to make sure that everyone has one and they are not left at home.
